Central City Integrated Health is a nonprofit organization in Detroit that provides health care, housing, and substance abuse services to members of the community. CCIH promotes high-quality care and housing for everyone, no matter their station in life. The organization, which offers comprehensive health services, housing support, and community re-entry services, depends on monetary donations to sustain its critical services. CCIH is an accredited Level III Patient-Centered Medical Home – the highest level of recognition awarded by the National Community for Quality Assurance (NCQA) – for its success in delivering team-based, integrated health care with the intent of obtaining maximum health outcomes for patients. The organization was originally founded in the late 1960s as part of President Lyndon Johnson’s Great Society program. It has provided whole health wellness services to the people of Detroit and Wayne County since 1972. CCIH served more than 4,400 individuals in 2018.
