Ann Arbor-based CareEvolution Inc. today announced an Apple Watch app that will connect patients and caregivers to the Trinity Health System, which includes Saint Joseph Mercy Health System and other hospitals across metro Detroit.

“Trinity Health’s strategy is to deliver people-centered care that improves patients’ experiences, improves the health of populations, and reduces the total cost of care,” says Barbara Walters, D.O., executive vice president for Trinity Health. “That includes innovations that make it easier to access and use health-related information.”

The app will connect patients to their health care providers, sending them health notifications, such as suggested preventative screenings, gaps in prescription refills, and information about potential drug interactions.

The app will be available as a free download from the App Store on the iPhone and is available to Trinity Health patients who are using the cFHR HealthKit iPhone app, which launched in December. The iPhone app must be downloaded onto the phone for the app to work on the Apple Watch. The Apple Watch will launch April 24.

The app was made in partnership with Anthem Inc., the parent company of Blue Cross Blue Shield.
