ConcertoHealth Expands Presence in Detroit, Opens New Clinical Administration Office in Southfield

California-based ConcertoHealth Inc., a provider of specialized primary care and supporting clinical services for vulnerable, frail, and elderly patients, announced today it will be opening a new administrative office in Southfield. The addition will serve as a clinical hub for the company’s primary care support services model in the Detroit metro region.

Located at 18000 W. Nine Mile Road, Suite 800, the new location will replace the previous office which was located in the New Center One building in Detroit.

“This new location provides a great opportunity to focus on patients’ needs and improve the quality of care in the Detroit metro region, while offering a collaborative space for primary and specialty care professionals to meet with patients and caregivers,” says Dr. Ernest L. Yoder, regional medical director for ConcertoHealth, Michigan. “Expanding our footprint in the Detroit area also means adding new team members who will serve our rapidly growing patient base, as well as support our health plan partners and their care providers.”

Additionally, ConcertoHealth operates a clinical hub in Kalamazoo and has four ConcertoHealth Care Centers in the Detroit metro area which offer 24/7 care coordination and support, and access to physicians, nurses, field-based clinicians, care coordinators, social workers, and pharmacists.

In combining its proprietary population health analytics platform, Patient3D, with on-the-ground care teams including social workers, care managers, pharmacists, field-based clinicians, and dietitians, the ConcertoHealth Primary Care Support Platform integrates clinical care with social services. Care settings can vary from at-home, to a doctor’s office, to a hospital, or post-acute setting.

The company has expanded its Detroit clinical hub team by 33 percent to 80 full-time employees since Oct. 2017. The new Southfield hub will house these team members, including care managers, and health plan and PCP support services staff.
