McLaren Macomb Hospital Launches In-home Support Program

McLaren Macomb Hospital, Medstar Ambulance, and Henry Ford Macomb Hospital have partnered to launch a program providing in-home support to recently discharged patients. Medstar paramedics will follow up with patients to help them remain on track with medications and physician directions.

“We all recognize that many of our treatment decisions and instructions may not be understood or followed. This program directly addresses those issues,” says Andrea Phillips, vice president and chief nursing officer at McLaren Macomb. “By engaging all of the stakeholders in the post-discharge experience, we identified frequent issues and challenges, which allow us an opportunity to make a difference.”

Phillips says adhering to physician directions and medications is key to patients’ continued improvement. She says the program was created to prevent patient readmissions and emergency room visits.

The Mobile Health Paramedic program is in a 180-day trial phase. McLaren Macomb Hospital, located in Mount Clemens, has nearly 300 beds and 400 physicians.

“We are thrilled to open our new state-of-the clinic, offering the same exemplary stand of medical care to patients in Livonia as we have been offering in West Bloomfield,” says Dr. Sanford Vieder, the medical director of both Lakes Urgent Care offices.

The new clinic, at 36622 Five Mile Rd., will be open every day of the year. It will also be home to a surgical center on the second floor, operated by St. Mary Hospital, and a magnetic resonance imaging center.
